Upward Health Partners with Alameda Alliance for Health to Provide Transitional Care Services

HAUPPAUGE, N.Y. / October 20, 2026 — Upward Health, a national provider of whole-person healthcare, today announced a partnership with Alameda Alliance for Health, a local health plan that provides health care coverage to Alameda County residents – to bring Transitional Care Services to adult patients at high-risk of inpatient readmissions. The program supports a Population Health Management requirement established by the California Department of Health Care Services under the California Advancing and Innovating Medi-Cal program, and is designed to reduce avoidable readmissions by surrounding patients with coordinated support before, during, and after a hospital discharge.

Patients have the best chance of avoiding readmissions when discharge planning is strong. Upward Health Transitions of Care Nurses meet patients in person at the hospital, coordinate the discharge process, and connect them to the clinical and community-based resources needed for a safe transition home. A key goal of the program is to ensure that a provider appointment happens within seven days of discharge, and Upward Health’s physicians, nurse practitioners, and behavioral health specialists directly coordinate this visit. A Transitions of Care Nurse works with patients for thirty days after discharge to ensure a successful return to the community and that there is engagement with existing or new primary care.

“The days right after a discharge determine whether a patient recovers at home or ends up back in a hospital bed,” said Dr. Anubhav Kaul, Chief Medical Officer, Upward Health. “That’s why our Transitions of Care Nurses start at the bedside, not with a phone call weeks later. Over the thirty days that follow, we reconcile medications, get patients in front of a primary care provider, and address the barriers at home that no discharge summary captures. Alameda Alliance for Health shares that commitment to meeting patients where they are, and we’re proud to extend their care team into the community.”

For local health plans, Transitional Care Services play a crucial role in supporting community health by reducing avoidable readmissions, lowering emergency department utilization, improving continuity of care after discharge, and meeting quality and regulatory goals. For health plans, Transitional Care Services support core performance and compliance priorities, including reducing avoidable readmissions, lowering emergency department utilization, improving continuity of care after discharge, and meeting quality and regulatory goals.

“This level of support is critical to helping reduce avoidable readmissions due to things like medication errors or inability to access their primary care provider,” said Dr. Donna White Carey, Chief Medical Officer at Alameda Alliance for Health. “Our hospital partners play a crucial role in caring for our members, and through this partnership with Upward Health, we are able to help our members safely transition from the hospital back into our community.”
