Santa Clara Family Health Plan partners with Upward Health Delivering in-home care management to Medi-Cal members
Santa Clara Family Health Plan (SCFHP) is excited to announce an expansion to its Enhanced Care Management network through a partnership with Upward Health.
Through Enhanced Care Management, a Medi-Cal benefit established by CalAIM (California Advancing and Innovating Medi-Cal), Upward Health will provide members with a single lead care manager to coordinate all their health and health-related care. Members with complex needs across Santa Clara County will benefit from getting the right care at the right time in the right setting.

These offerings are customized by Upward Health for SCFHP members who frequently use urgent or intensive healthcare services and have complex needs, including serious mental illness, substance use disorder, and housing instability. Services are provided in members’ homes or preferred location and focus on building trusted relationships, coordinating physical, behavioral, and social services, and addressing barriers that often prevent individuals from engaging in traditional care settings.
