Upward Health Partners with Aetna to Serve Patients in California
We’re excited to share that we expanded our partnership with Aetna, a CVS Health Company, into California, growing from our collaboration in Arizona. We have been serving patients in California for a number of years, and we’re happy to leverage our experience in the state for our partners at Aetna.
Together, we’re bringing our whole-person, in-home care model to Aetna Medicare Advantage members living with multiple chronic conditions. Our approach delivers personalized, hands-on support that helps patients stay healthier in the community and avoid unnecessary time in the hospital.
Services include:
- In-home visits from doctors and nurses
- 24/7 access to medical professionals
- Support transitioning home after hospital stays
- Help addressing social needs like transportation and food access
