Connecticut is a natural extension of our existing New York market, allowing us to build on regional infrastructure and relationships while expanding care to new communities.
The state includes a strong population of patients facing complex physical, behavioral, and social challenges. These characteristics make Connecticut well aligned with Upward Health’s whole-person approach to care.
Together, these factors made Connecticut a natural next step in expanding our presence in the Northeast.
Upward Health’s model is designed for rapid market entry when health plan partners need support. In Connecticut, we mobilized quickly to stand up operations and began serving patients within months.
At launch, our teams were positioned across all eight counties, enabling us to support patients in both urban and rural communities throughout the state.
Our presence includes established relationships with major health systems and provider groups, along with connectivity to the Connecticut statewide health information exchange to ensure strong patient data visibility from the outset.
Because our care is delivered directly in patients’ homes and communities, our model does not rely on building or operating traditional healthcare facilities. This allows us to deploy quickly while maintaining the high-touch, whole-person care patients with complex needs require.
